Sacramental Records 

If you received the sacrament of Baptism, First Communion, Confirmation or Marriage at St Francis of Assisi, we are happy to supply a copy of your sacramental record.
 
Please use the form here to request your sacrament record.
  1. You can answer the questions into an email and send it to [email protected]
  2. You can print, fill out and submit your request in person or mail to the parish office.
  3. Or stop by the parish office and fill out the request.  

Please allow at least 5-10 business days for us to research our files and process your request, plus any additional time it may take for the mailing. Same day request is not available.
