Child must be Baptized in a Catholic Church.  Be in Second Grade or older.  Registration form and Medical Release are required by Monday, September 28th, 2008.  All Candidates must use the Registration Form from this page!

• If the Candidate is a registered student at St. Francis Parish School then the Sacrament Preparation is taught in the Classroom during the School Day. 

• If the Candidate is in Public School and attends Wednesday Religious Education (CCD) classes  the Sacrament Preparation is taught during the Religious Education course on Wednesday afternoons.   (by registering for the Sacrament you will be automatically registered for Wednesday RE classes.)

• If the Candidate's parent wishes to "Home School" the Sacrament special arrangements may be made with a conference with Duke Johnson, Director of Religious Education.
